I have swapped some devices that failed with new. PH appears to be in an endless loop. The raw log shows "Insteon NAK <address>", then a TX and 2 RX over and over. There are many pending operations that are not getting done.

I have restarted PH, when I go into Insteon Maintenance it starts this again.

Running PH v2.1.4

Any ideas?

For a starter you might hard reset the PLM. Disable any macros that startup automatically including timed events and see if the symptoms change. Need to get closer to the problem.

I have done a PLM Clear and Full load, and a rebuild of the PLM. But will also try the "hard reset".

Question: what causes an Insteon NAK? Is it a problem with the device (a switchlinc), the PLM, or something else?

Will update after the hard reset.

The quick answer is all of the above. Noise on the line can cause a NAK. Someone is reporting they did not get a response so if you know who is doing the reporting then you normally would assume is outbound from there. Am I to assume what you are calling Insteon Maintenance is Insteon Explorer? If so uncheck Enable Pending off if on and see if it stops. The fact that PH appears to be hung in a loop would tell me the PLM is hung. But that is a guess.

If I understand the initial post, some new devices have been installed which are giving endless NAKs. Just about every device Smarthome is now shipping is an I2CS device. There is a special I2CS Zip that Dave has posted that must be installed on top of 2.1.4. Once that is done the I2CS device install must complete a Status Scan for the I2CS install process to complete.

I2CS devices have additional security built in that prevent PLMs the device is not aware of from controlling and writing to the device. The device NAKs requests. Also I2CS devices have an additional requirement for a CheckSum to be included in extended messages. The I2CS support Zip has the code to deal with both of these requirements.
