Has anyone ever experienced a SwitchLinc turning itself off?

I have one light that suddenly seems to be going out on its own; I'm pretty sure there is nothing programmatic that I've done to cause it to happen.

While I have PH turn the light on, anywhere from a few seconds to 10 minutes after the light gets turned on, I see the following in the PH logs:
Incoming Insteon received on Insteon DM controller PLM. From Address:[0C.71.FE] To Address:[00.00.01] Flags:[203] Cmd1:[19] Cmd2:[0]. Decode: Group Broadcast. From: FRONT PORCH Group: 1, Off

The 0C.71.FE address is the address of that light.

The last entry in the log is:
Incoming Insteon received on Insteon DM controller PLM. From Address:[0A.AC.5F] To Address:[0F.D5.22] Flags:[107] Cmd1:[17] Cmd2:[4]. Decode: ACK of Group Cleanup. From: KPL FIREPLACE To: PLM Group: 4, On

KPL Fireplace 4 is linked to the light that is turning off. So my KPL button press turns the light on, and then some time passes, and then the light turns off.

Any ideas on whether this is something other than hardware failure?

The following message indicates the Off paddle was pressed on 0C.71.FE. It is a Group Broadcast for Group 1 with an OFF command which is what you get when the Off paddle is pressed. The only way I know of for this message to be issued is from an actual paddle press (or hardware problem). Perhaps the OFF paddle tact switch is marginal. Check the paddle mounting to be sure the paddle is not binding on the frame and is free to move away from the OFF position.
